Рак предстательной железы (РПЖ) по итогам за 2019 г. является второй наиболее часто диагностируемой злокачественной опухолью у мужчин, проживающих в Российской Федерации. Отмечается значительная доля больных с запущенными (III–IV стадиями) РПЖ – до 50 %, при этом на метастатический рак приходится до 18,1 % больных. Популяризация в клинической практике московского городского здравоохранения инновационных технологий ведения пациентов с метастатическим кастрационно-резистентным раком предстательной железы (мКРРПЖ) напрямую отвечает целям и задачам Федерального проекта, направленного на увеличение продолжительности жизни онкологических больных.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В статье рассмотрены итоги клинических разработок в области лечения мКРРПЖ, представленные на Ежегодном конгрессе Американского научного общества клинической онкологии в период с 3 по 7 июня 2022 г. (The ASCO Annual Meeting, June 3–7, 2022): под-исследования VISION, исследование TheraP, изучение прогностической роли циркулирующей опухолевой ДНК (ctDNA) в раннем выборе стратегии применения абиратерона или энзалутамида у больных с исходным мКРРПЖ – treatment naïve-статусом.</p></sec><sec><title>Результаты и обсуждение</title><p>Результаты и обсуждение. Объем клинической информации по результатам использования тераностических противоопухолевых лучевых технологий, реализуемых с участием простат-специфического мембранного антигена (далее – PSMA), стремительно растет. PSMA – опосредованная радио-визуализация рассматривается как потенциально эффективный инструмент отбора пациентов, чувствительных к специфической терапии. При изучении радионуклидной терапии, опосредованной лигандом к PSMA (PSMA-Л), основные усилия фокусируются на определении оптимальных режимов ее клинического применения. Также изучаются возможности расширения текущих показаний для назначения 177Lu-PSMA-617 пациентам с РПЖ на более ранних стадиях. До получения финальных результатов по клиническим испытаниям, дающим четкие ответы на открытые вопросы относительно различных аспектов применения инновационных PSMA – опосредованных тераностических технологий, остается очень важным достижение экспертного консенсуса, в рамках обмена мнениями, относительно их надлежащего использования в различных клинических ситуациях. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Prostate cancer (PC), according to the results for 2019, is the second most frequently diagnosed malignant tumor in men living in the Russian Federation. There is a significant proportion of patients with advanced (III-IV stages) of prostate cancer - up to 50 %, while metastatic cancer accounts for up to 18.1 % of patients. Popularization in the Moscow City Healthcare clinical practice of innovative technologies for managing patients with metastatic castration-resistant prostate cancer (mCRPC) directly meets the goals and objectives of the Federal project aimed at increasing the life expectancy of cancer patients.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The article reviews the results of clinical developments in the field of mCRPC treatment presented at The ASCO Annual Meeting, June 3-7, 2022: VISION sub-studies, TheraP study, study of the prognostic role of circulating tumor DNA (ctDNA) in the early choice of a strategy for the use of abiraterone or enzalutamide in patients with initial mCRPC - treatment naïve - status.</p></sec><sec><title>Results and discussion</title><p>Results and discussion. The volume of clinical information on the results of theranostic antitumor radiation technologies implemented with the participation of prostate-specific membrane antigen (hereinafter referred to as PSMA) is growing rapidly. PSMA - mediated radio-imaging is considered as a potentially effective tool for selecting patients who are sensitive to specific therapy. When studying radionuclide therapy mediated by a ligand to PSMA (PSMA-L), the main efforts are focused on determining the optimal regimens for its clinical application. The possibility of expanding the current indications for prescribing 177Lu-PSMA-617 to patients with prostate cancer at an earlier stage is also being studied.
