В результате охватившей весь мир пандемии COVID-19 в 2020 году изменились уровень и структура заболеваемости, «центр тяжести» смертности взрослого населения сместился в сторону инфекционных заболеваний. Противоэпидемическая переориентация систем здравоохранения в мире оказала негативное воздействие на здоровье пациентов с неинфекционными заболеваниями. Вследствие этого заболеваемость взрослого населения, в частности болезнями органов дыхания, относящимися к важнейшим неинфекционным заболеваниям, приобретает особую актуальность в условиях эпидемиологических ограничений в городе Москве при распространении новой коронавирусной инфекции COVID-19 и возросшей нагрузке на систему здравоохранения столицы.</p></sec><sec><title>Цель</title><p>Цель. Анализ показателей общей и первичной заболеваемости по классу болезней органов дыхания взрослого населения города Москвы и Российской Федерации за периоды 2015–2019 гг. и 2019–2020 гг.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В качестве материалов были использованы данные Минздрава России (заболеваемость по данным обращаемости, отчетная форма ФСН № 12) за 2015–2020 гг. по классу болезней органов дыхания и отдельным нозологиям, относящимся к хроническим болезням нижних дыхательных путей: астма, астматический статус (J45, J46); другая хроническая обструктивная легочная болезнь (J44); хронический бронхит, эмфизема (J40-J43).</p></sec><sec><title>Результаты</title><p>Результаты. Болезни органов дыхания продолжают занимать лидирующие позиции в структуре заболеваемости взрослого населения города Москвы и Российской Федерации. В структуре общей заболеваемости взрослого населения города Москвы в 2020 году болезни органов дыхания составляли 16,8 % (в 2015 г. – 15,1 %), что ниже, чем в целом по РФ – 17,7 % (2015 г. – 13,8 %). В структуре первичной заболеваемости взрослого населения города Москвы они составляли 33, 8% (в 2015 г. – 33,5 %), что ниже, чем в целом по РФ – 36,9 % (2015 г. – 28,2 %). Общая заболеваемость взрослого населения по классу болезней органов дыхания в 2020 г. в Москве составляла 20 830,8 на 100 тыс. населения, что на 19,6 % ниже, чем в целом по РФ (25 910,0). За период 2015–2019 гг. показатель общей заболеваемости имел тенденцию к росту в Москве на 10,2 %, в РФ – на 6,2 %. За период 2019–2020 гг. в Москве прирост показателя составил всего 5,4 %, в РФ отмечались более высокие темпы прироста – 21,9 %. Первичная заболеваемость в 2020 г. в Москве составляла 15 832,1 на 100 тыс. населения, что в 1,3 раза ниже среднероссийских показателей (21 376,0). За период 2015–2019 гг. уровень заболеваемости снизился в Москве на 7,9 %, в РФ – на 6,6 %. В 2019–2020 гг. темп прироста данного показателя в Москве составил 4,8 %, в целом по РФ зарегистрирован более высокий прирост – 29,7 %. За период с 2015 по 2019 г. и в период «ковидных ограничений» 2020 года преимущественно в Москве отмечаются высокие темпы прироста первичной заболеваемости хронической обструктивной болезнью легких и астмой. В то же время зарегистрировано снижение уровня общей и первичной заболеваемости хроническим бронхитом и эмфиземой как в столице, так и в России в целом.</p></sec><sec><title>Заключение</title><p>Заключение. Таким образом, заболеваемость взрослого населения как по классу болезней органов дыхания, так и по отдельным нозологиям в г. Москве имеет тенденцию к снижению, в то время как в целом по Российской Федерации – к росту, что требует проведения дополнительных исследований для нахождения причинно-следственных взаимосвязей.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. As a result of the COVID-19 pandemic that swept the whole world in 2020, the level and structure of morbidity changed, the “center of gravity” of adult mortality shifted towards infectious diseases. The anti-epidemic reorientation of healthcare systems in the world has had a negative impact on the health of patients with non-communicable diseases (NCDs). As a result, the morbidity of the adult population, in particular respiratory diseases related to the most important NCDs, becomes particularly relevant in the conditions of epidemiological restrictions in Moscow with the spread of the COVID-19 and the increased burden on the Healthcare system of Moscow.</p></sec><sec><title>Purpose</title><p>Purpose. The purpose of this study is the analysis of indicators of incidence and prevalence of the respiratory system diseases of the adult population of Moscow and the Russian Federation for the periods 2015–2019 and 2019–2020.</p></sec><sec><title>Methods and materials</title><p>Methods and materials. The study used data of the official statistics of the Ministry of Health of the Russian Federation of the “Morbidity of the Russian population” (incidence and prevalence by the care seeking data) for 2015–2020, by the ICD-10 chapter X “Diseases of the respiratory system” (J00-J99), blocks “Chronic lower respiratory diseases” (J40-J47): Asthma, Status asthmaticus (J45, J46); Other chronic obstructive pulmonary disease (J44); chronic bronchitis, Emphysema (J40-J43).</p></sec><sec><title>Results</title><p>Results. The respiratory system diseases continue to occupy leading positions in the structure of morbidity of the adult population of Moscow and the Russian Federation. In the structure of the prevalence of the adult population of Moscow in 2020, respiratory system diseases accounted for 16.8 % (in 2015 – 15.1 %), which is lower than in the whole of the Russian Federation – 17.7 % (2015 – 13.8 %). In the structure of the incidence of the adult population of Moscow was 33.8% (in 2015 – 33.5 %), which is lower than in the whole of the Russian Federation, amounting to 36.9 % (2015 – 28.2 %).</p><p>The prevalence the respiratory system diseases of the adult population of respiratory system diseases in 2020 in Moscow was 20 830.8 per 100 000 population, which is 19.6 % lower than in the whole of the Russian Federation (25 910.0). Over the period 2015–2019, the indicator of prevalence tended to increase in Moscow by 10.2 %, in the Russian Federation – by 6.2 %. For the period 2019–2020 in Moscow, the increase was only 5.4 %, in the Russian Federation there were higher growth rates (+21.9 %).</p><p>The incidence the respiratory system diseases in 2020 in Moscow was 15,832.1 per 100 000 population, which is 1.3 times lower than the national average (21,376.0). Over the period 2015–2019, the incidence rate decreased in Moscow by 7.9 %, in the Russian Federation – by 6.6 %. In 2019–2020, the growth rate in Moscow was 4.8 %, in general, a higher increase of 29.7 % was recorded in the Russian Federation.</p><p>For the period from 2015 to 2019 and during the period of “COVID-19 lockdowns” in 2020, mainly in Moscow, there are high rates of increase in the incidence of chronic obstructive pulmonary disease and asthma. At the same time, a decrease in the level of prevalence and incidence of chronic bronchitis and emphysema was registered both in Moscow and in the Russia.</p></sec><sec><title>Conclusions</title><p>Conclusions.
