Введение. Доступность медицинской помощи является одним из показателей качества медицинской помощи. Ежедневно при работе врачей отмечается неявка пациентов на прием без отмены записи. Это вносит существенную роль в ограничение доступности медицинской помощи.
Цель. Проанализировать показатели несостоявшихся приемов у детского хирурга в течение 2021 года в поликлиниках Северо-Восточного административного округа города Москвы.
Материалы и методы. Проведена оценка записей к врачу – детскому хирургу в течение 2021 года в четырех детских поликлиниках Северо-Восточного административного округа города Москвы. Несостоявшиеся приемы разделены на группы в зависимости от времени записи, возраста пациента, пола ребенка и способа записи, проведен ретроспективный анализ данных групп.
Результаты. Определена общая доля несостоявшихся приемов по профилю «детская хирургия» в Северо-Восточном административном округе города Москвы. Лица мужского пола чаще записывались к хирургу ‒ из 84 459 в 48 718 случаях (57,4 %), лица женского пола ‒ в 35 741 случае (42,6 %). Доля неявок составила 18 % и 17 % соответственно, однако различия недостоверны (р=0,25). При анализе неявок относительно возраста пациентов отмечено, что минимальную часть неявок имеют дети от 0 до 3 лет – 13,4 %, а максимальную дети 6–13 лет – в 21 % случаев (различия достоверны, р=0,001). Также было установлено, что максимальная доля неявок на прием относительно времени записи приходится на 16–20 часов – 20 %, а минимальная – на время с 8 до 12 часов, что составляет 15 % случаев (р=0,001).
Выводы. Информирование о возможности отмены записи к врачу интенсивней должно вестись в группе родителей школьников. Внедрение правила уточнения у законных представителей явки на прием по телефону при записи на вечернее время может способствовать снижению доли неявок. Отмечен важный вклад пациентов и их законных представителей в формирование такого важного показателя в работе медицинских организаций, как доступность.
Introduction. The availability of medical care is one of the indicators of the quality of medical care. Every day, during the work of doctors, there is a non-appearance of patients for appointments without canceling the appointment. This plays a significant role in limiting the availability of medical care.
Purpose. To analyze the indicators of failed appointments with a pediatric surgeon during 2021 in polyclinics of the North-Eastern Administrative District of Moscow.
Methods and materials. The evaluation of records to a pediatric surgeon during 2021 was carried out in four children's polyclinics of the north-eastern district of Moscow. Failed receptions were divided into groups depending on the time of recording, the age of the patient, the sex of the child and the method of recording, a retrospective analysis of these groups was carried out.
Results. The total share of failed appointments in the field of «pediatric surgery» in the North-Eastern Administrative District of Moscow was determined. Males were more often registered with the surgeon – out of 84 459 in 48 718 cases (57.4 %), females – in 35 741 cases (42.6 %). The proportion of no-shows was 18 % and 17 %, respectively, but the differences are not significant (p=0.25). When analyzing absenteeism in relation to the age of patients, it was noted that children from 0 to 3 years of age have the minimum part of absenteeism – 13.4 %, and children aged 6–13 years old – in 21 % of cases (significant differences p=0.001). It was also found that the maximum share of non-attendance at appointments relative to the recording time falls on 16–20 hours – 20 %, and the minimum – from 8 to 12 hours, which is 15 % of cases (p=0.001).
Conclusions. Informing about the possibility of canceling an appointment with a doctor should be carried out more intensively in a group of parents of schoolchildren. The introduction of a rule to clarify with legal representatives about attendance at an appointment by phone, when making an appointment in the evening, may result in a decrease in the proportion of absences. The important contribution of patients and their legal representatives to the formation of such an important indicator in the work of medical organizations as accessibility was noted.
