Abstract

Introduction. Currently, the incidence of kidney diseases among adult population is growing. It is expected to become the fifth leading cause of death by 2040. Showing no symptoms at early stages, chronic kidney disease (CKD) is closely related to diabetes mellitus, hypertension, obesity and kidney dysfunction in older age, which requires long-term and expensive treatment. Organization structure and the strategy of medical care provided by Moscow nephrology service is aimed at implementing measures for risk prevention and active early detection of kidney disease, as well as informing general population and medical community about existing diagnostic methods, modern treatment options and schemes.

The purpose of the study was to identify trends in CKD prevalence and incidence among able-bodied population in the context of Moscow nephrology service improvement.

Materials and Methods. An analysis of disease prevalence and incidence among able-bodied population based on official statistics data of the Russian Research Institute of Health (RIH) of the Ministry of Health of the Russian Federation and the Centre of Medical Statistics of Moscow Healthcare Department was carried out; indicators of time series and average growth/decrease rates were calculated. In order to identify a long-term trend, least squares method was used. Incidence rates were expressed per 100,000 able-bodied population.

Results. In 2022, the overall disease prevalence in Moscow was lower than for the country as a whole (31.3 and 74.2 cases per 100,000 population, respectively), while incidence of the able-bodied population in Moscow was lower than countrywide (2.9 and 11.5 cases per 100,000 population, respectively). The long-term dynamics of CRD prevalence and incidence was generally characterized by an upward trend. Thus, the average annual incidence in Moscow was 9.8% and 6.1%, while in the Russian Federation 7.2% and 5.4%, respectively. On average, 91.3% of able-bodied patients with CKD in Moscow are under follow-up monitoring for a year. The number of dispensary patients with CKD increased by 2.5 times in 2022 in comparison to 2013, from 11.7 to 29.5 per 100,000 population, respectively. Since 2016, patients with newly diagnosed CKD have been provided with 100% coverage of follow-up monitoring.

Abstract

Introduction. The concept of salutogenic design, focused on creating a healthy environment, is based on the salutogenesis concept, developed back in the 1970s by Aaron Antonovsky. This approach is becoming increasingly popular in urban planning, development, and certification of buildings, offices, and medical facilities. Due to ongoing national projects, there is an increasing demand for specialists in public health and salutogenic design in Russia, which has justified the creation of an interdisciplinary educational program.

Objective. To present the implementation of the network master's program "Salutogenic Design" in the field of public health and the creation of a healthy urban environment.

Materials and methods. We conducted literature review on existing practices for applying salutogenic design in urban planning and described the implementation of the network master's program "Salutogenic Design" at the Institute of Leadership and Health Management of the I.M. Sechenov First Moscow State Medical University (Sechenov University) and Moscow institute of architecture (MIARCH).

Results. The concept of salutogenic design is a prominent scientific and practical issue for researchers, professional community, and executive authorities worldwide, with particular interest in implementation studies of salutogenic design for designing and constructing environments, transport, medical organizations, and the urban landscape, as well as developing tools for urban environment assessment. The master's program "Salutogenic Design" is a unique educational experience that includes the development of professional skills in an interdisciplinary area of health sciences and urban planning, distinguishing it from other educational projects.

Abstract

Introduction. Currently, there is a challenging issue related to adaptation of young doctors to professional activities in healthcare organizations. The first years of work are considered as of one of the most difficult stages of a doctor's career, and the decision to continue clinical practice or give up it largely depends on this period. This, in turn, is one of the reasons for the increase in staff turnover, which negatively affects the activity of a medical organization and the quality of medical care.

Purpose. The purpose of this study was to develop proposals for optimizing the process of adaptation of young doctors to professional activities in medical organizations upon the results of a study identifying factors that positively and negatively affect it.

Methods and materials. Within the framework of this study, a questionnaire survey with participation of young doctors aimed at identifying factors affecting the process of adaptation to professional activity was conducted. Also, a social-hygienic image of a young specialist, motivation, reasons for career choice were analyzed, as well as the most common challenges emerging at the beginning of career and during the studies were identified, evaluated and compared.

Results. Our research allowed us to identify the most important factors influencing the adaptation process such as convenient work schedule; competitive salary; friendly working environment; modern medical equipment; image of a medical organization; opportunities for professional training; career growth opportunities; job stability; job independence and responsibility and creative work. The results show that the adaptation of a young doctor to professional activity in healthcare organizations is crucial to keep human and economic resources in balance. The existing programs are not as efficient as expected. Young specialists note that among the difficulties affecting the quality of medical care are such as lack of knowledge and skills required for their job. Moreover, lack of specialized programs for professional training concerning medical information system and legal and regulatory framework in healthcare, as well as lack of psychological support services leads to staff turnover.

Abstract

Introduction. Recently, the medical tourism industry in Thailand has undergone significant qualitative transformations, with the healthcare sector as the main driver of the development and growth of the country's economy. On a global scale, medical tourism in Thailand demonstrates a significant increase in market value that exceeds the global average, ranking in the top five of the most attractive medical tourism destinations.

A combination of medical services provided by highly qualified specialists, lowcost treatment, as well as ambitious government initiatives allowed Thailand to become one of the industry leaders and a significant hub for healthcare and tourism development. In general, there is a significant potential for market expansion. In particular, Russian tourists are largely represented in Thailand’s tourism market.

Purpose. The purpose of the study was to identify the key drivers of medical tourism in Thailand that may help to adjust Russian national strategy to the development of medical tourism and the export of medical services within the framework of the Federal Project ”Export of Medical Services” as part of the National Project “Healthcare” upon the Decree of the President of the Russian Federation No. 204 dated May 7th, 2018.

Results and conclusion. Despite the fact that modern international medical tourism is relatively new, the entry into the market of both a separate medical organization and a governmental one is associated with a high threshold of entry requiring significant legal, infrastructural, scientific, and educational transformations, as well as ensuring the safety of medical tourism services at different levels, from national to municipal and hospital. Thus, the global scientific society is interested in the experiences of the countries that have achieved significant results in the international medical tourism market. Thailand is a striking example of a leading destination for medical tourists from Southeast Asia. The ecosystem of medical and wellness tourism created and supported by the government includes doctors, patients, medical organizations, international certification companies, assistance agencies, and medical tourism agencies. The creation of appropriate conditions for the essential development of all market participants allowed Thailand to begin its journey as a global medical destination in the Eastern Hemisphere.

Abstract

Purpose. The article discusses the improvement of e-Health training of medical specialists in Russia, and the challenges that doctors face when working with different health information systems.

Methodology. The study used both qualitative and quantitative data of the online survey conducted during the primary case study. Online survey targeted students of the Department of Medical Informatics and Cybernetics of N.I. Pirogov Russian National Research Medical University and doctors who were receiving initial training under the e-Health advanced training program. The survey aimed to determine systemic challenges and demand for training in various medical information systems.

Findings. Research data suggest that in order to bridge the digital gap, educational and training programs should include training versions of medical information systems used in Russian practical healthcare (electronic document flow system, EMIAS, Statistica, Sotsstrakh, Retsipient, etc.). It will enable doctors to enhance their theoretical and practical skills, abilities and knowledge, thus improving the quality of educational and work processes. Practicing health specialists who not only possess the theoretical knowledge, but also actively work in these areas (temporary disability examination, evaluation of medical care quality, work with citizens' appeals, compilation of numerous statistical reports, etc.) should be involved as mentors in the educational process. Simultaneous connection under the same login and password from multiple devices should be restricted to protect personal data and increase the security level of electronic document management systems in Russia.

Originality/value. These findings could greatly influence e-Health training of Russian doctors and minimize the risks associated with erroneous use of health information systems.

Abstract

Background. The Russian national project “Healthcare” is aimed at providing the healthcare system with qualified doctors. Higher education institutions have a leading role in accomplishing such crucial state mission. The training of future doctors by medical universities is complicated by numerous medical and social issues associated with objective living conditions and subjective lifestyle as well as with a low motivation to become a doctor among students and residents.

Purpose. To conduct a medical and social analysis of living conditions, lifestyle, and motivation to become a doctor among students and residents of medical university.

Materials and methods. The research was based on the data of a medical and sociological analysis carried out from 2020 to 2023. The authors used sociological, psychological, and statistical techniques that are relevant to the study purpose and enable to assess living conditions, lifestyle, and motivation to become a doctor.

The study subject was a representative sample (p<0.05) of 1007 students and residents at a medical university.

Results. The living conditions of students and residents are considered as ambiguous, but generally favorable for achieving specialized learning and career-related outcomes at medical university. The motivation to become a doctor is affected more by the families of students and residents and less by the objective living conditions and subjective perceptions of lifestyle. The motivation has a positive modality, pronounced intensity, and pragmatic orientation.

Conclusions. The professionalism of future doctors is usually developed depending on their motivation and in the context of their current living conditions at medical university. The shaping processes could be optimized by using suitable approaches to educational work and social support for students and residents.

Abstract

Introduction. A healthy lifestyle is a lifestyle aimed at preventing emergence and development of non-communicable diseases characterized by exclusion or reduction of behavioral risk factors. Adherence to the principles of healthy lifestyle among medical workers abroad is of great importance. Therefore, it is relevant to study foreign experience related to adherence to healthy lifestyle among medical professionals (doctors and nursing staff), both in routine practice and during the COVID-19 pandemic.

Materials and methods. During a search for the keywords “healthy lifestyle”, “medical workers” and “pandemic” in the period from 2014 to 2023 conducted in the PubMed international database, more than a thousand of systematic reviews, meta-analyses and articles on relevant scientific and practical topics were found. Results of interest included occupational health and health behavior of medical workers. After the final selection and exclusion of the results that are scientifically and practically insignificant, 30 sources from 27 countries were included in the analysis. The authors assessed the quality of these reviews, meta-analyses and articles and obtained new summarized data on the topic.

Results and discussion. The article presents analysis and summary of 30 systematic reviews, meta-analyses and articles published from 2014 to 2023 by authors from 27 countries. For this purpose, the results of an analysis of adherence to healthy lifestyle among medical workers, the relationship of lifestyle and health-related behavior; lifestyle, shift work and health; lifestyle and disease development in routine practice as well as during the COVID-19 pandemic were presented.

Conclusion. The correlation of lifestyle and health-related behavior of medical workers; lifestyle, shift work and health; lifestyle and disease development in routine practice as well as during the COVID-19 pandemic were revealed. Optimizing recommendations suggesting necessary and appropriate measures to improve the adherence to healthy lifestyle among medical workers were elaborated.

Abstract

Introduction. Healthcare workers are an important part of national healthcare systems ensuring its sustainability. Thus, it is necessary to introduce state mechanisms aimed at protecting their health and ensuring safe working conditions. Moreover, protection of rights and interests of healthcare workers is a matter of social justice. International and national public organizations and especially trade unions of healthcare workers play an important role in dealing with these issues.

The purpose of the study is to consider examples of trade union activities on health protection and provision of safe working conditions in a number of countries upon the data on health issues that experience medical specialists as well as factors influencing these issues.

Materials and methods. A content analysis of the official Internet pages of trade union organizations and their reporting documents for 2015-2024 was performed. Also, such methods as analysis, synthesis, secondary analysis and systematization of data were used.

Results. Around the world, healthcare workers face occupational risks of infectious diseases, and are constantly exposed to various types of biological and non-biological hazards due to their working conditions, which are considered unsafe. Even cases of ill-treatment, threats, attacks or insults during the performance of professional duties are documented. Therefore, protection of health and safety of medical professionals, as well as their social and labor rights and interests is one of the main areas of trade unions as public organizations created on the initiative of healthcare workers.

Abstract

In this review, organizational technologies for early cancer detection including screening programs and patient information management systems are discussed. An analysis of social technologies applied to early cancer detection is carried out, and the role of modern information technologies in cancer control management is discussed. The purpose of the study was to review and analyze existing organizational and social technologies used for early cancer detection, as well as for effectiveness evaluation.

Abstract

Background. Timely detection and treatment of diabetic retinopathy (DR) could prevent the development of blindness. However, large number of patients are diagnosed at late stages when effective treatment becomes impossible. In this context, active screening for DR plays an important role. Various screening programs have been developed, including programs using digital technologies.

Objective. To analyze and summarize the literature data on digitalization of DR screening.

Materials and methods. The study was carried out based on the results of a search in the PubMed / Medline bibliographic database and the Google system. The used keywords were “diabetes”, “retinopathy”, “telemedicine”, “digital technologies”, “artificial intelligence” and others. Relevant publications were selected by examining the titles and abstracts of articles.

Results. The study addressed the approaches to organizing the telemedicine programs of DR screening. The examples of innovative digital technologies for performing diagnostic studies as well as the characteristics of various electronic systems and devices are presented. It was noted that the DR screening could be performed in a local center with data transfer to a regional specialized diagnostic (analytical) center where telemedicine analysis is done. All diagnostic information is available to the diabetologist who prescribes and adjusts therapy. This operating principle facilitates the interaction between various specialists and thus increases the efficiency of screening.

Discussion. Although the telemedicine screening for diabetic retinopathy has already been introduced in a number of countries, it is still at its early stages of development. Many modern technologies such as portable mobile devices could be adapted and used in the implementation of telemedicine programs. Deep machine learning algorithms and other artificial intelligence-based approaches demonstrate excellent results. Some systems have already received regulatory approval.

Conclusions. The early diagnostics of diabetic retinopathy could be improved by integrating innovative screening technologies in a unified digital platform implemented in routine clinical practice.

Abstract

Introduction. Studying the evolution and life cycle of topics that form a particular field of knowledge is important in order to predict certain trends in the development of scientific research in any scientific field (including medicine), as these topics are crucial in identifying and developing new directions. The SciVal topic cluster system is one of the effective, appealing, and promising models for topic-based article classification. It is utilized on the SciVal analytical platform using data from the Scopus international scientific citation database.

Materials and methods. Data collection and uploading were carried out as of January 24, 2023. The time frame of the study was four years, from 2019 to 2022. The goal was to examine a set of medical publications for 15 organizations (4 research institutes and 11 scientific and practical centers) subordinate to Moscow Healthcare Department.

Results and discussion. Scientific organizations were ranked according to six scientometric indicators that describe the share distribution and quantitative characteristics of medical topic clusters. The study determined the leaders among all the 15 organizations as well as a top three among research institutes and scientific and practical centers. Topic clusters were analyzed based on the maximum and minimum values in the number of publications and citation indexes across 15 organizations and separately for two groups (research institutes and scientific and practical centers).

Conclusion. One of the primary paths of future development and implementation of the methods presented in this paper appears to be the use of matrix analysis to identify and determine the key features of both individual subject clusters and joint thematic clusters. With the use of matrix analysis, we will be able to establish the most extensive, referenced, and relevant topic clusters based on the quantitative and equity ratios that we calculate not only between the topic clusters themselves but also between organizations that have publications in these clusters.

Abstract

Introduction. Modern medical organizations, multifunctional medical centers, public and private medical institutions actively use digital technologies such as document management systems to optimize internal business processes and increase the efficiency. Proper document management and easy access improve the workflow. Thus, integration and development of advanced digital products allow medical organizations to ensure efficient time management which, in turn, reduces costs and saves time.

The purpose of the study was to present theoretical aspects of digitalization of document management system in medical organizations as a modern organizational and managerial process that ensures the effectiveness of healthcare development.

Materials and methods. Content analysis of scientific literature on the development and implementation of document management systems, transfer of documents to electronic version, introduction of digital technologies upon the specifics of medical organizations was used.

Results. The study showed that document management systems currently run up document-based information lifecycle management. Their active implementation began in the 2000s, and it is expected that the market volume of document management systems will grow from 6.57 billion US dollars in 2023 to 12.94 billion US dollars by 2028, on average by 14.50% during the forecast period (2023-2028). Significant amounts of health data are collected manually using various internal IT-systems and numerous collections of documents, databases and forms. The introduction of document management systems allows healthcare actors to create electronic patient records, thereby minimizing the risk of documentation loss and increasing security. It is assumed that in the near future all the system will be based on digital solutions aimed at solving routine tasks such as document management, storage and use. These trends provide solutions to the problems that medical organizations may face and ensure effective strategies based on the digitalization of document management systems.

Abstract

The author carried out the mathematical modeling of the longevity index among the famous mathematicians who had most of active life years during the existence of the Soviet Union. Based on a sample of statistical data, the probability distribution and density functions for the longevity of mathematicians who were born or worked in the Soviet Union were created. Modeling was done using polynomial functions through the Excel regression analysis tool. A comparative analysis of the probability distribution functions for the longevity of famous Soviet mathematicians and mathematicians who lived in different countries in the 20th century was performed.
